Section of Clinical Neuropharmacology, Division of Psychological Medicine, Institute of Psychiatry, De Crespigny Park, London A case of heatstroke is reported in a 32-year-old man diagnosed with schizophrenia and on clozapine monotherapy. The importance of the need to avoid misdiagnosing heat stroke as neuroleptic malignant syndrome is reviewed.
